There are several quests that sometimes or always glitch. The developers have fixed all quests up to party level 250. We must have patience for the ones above that and wait for a future update to fix this. However, you can use some of these quest glitches to your advantage. Their are several quests where the monsters kill themselves, helping your party to achieve high xp rates on quests you may have otherwise found hard.
